Hello student,

Please fill out the following form by Thursday, April 18th at 11:59pm.

This form will ask for your attendance on May 6th (Compliance Day) and May 9th (BLS Day).  Compliance Day on May 6th, 2024, is mandatory for students who have not completed their health clearance or CastleBranch uploads by May 6th. BLS day is mandatory for students who do not have their BLS certification by May 9th.

*As a reminder, if you do not complete the program requirements by the deadline outlined in the information packet (sent to you March 26th) you will not be allowed to continue with the program.
