📝 ATI TEAS Exam Reservation Form

• Exam Date: Monday, November 16, 2026
• Exam Format: In Person
• Arrival and Check-In: 8:30 a.m.
• Exam Start Time: 9:00 a.m.
• Location: ATAS 233 (second floor) at Saddleback College Campus
• Parking: Students may park in Lot 5, Lot 5A, or Lot 7
• Please allow enough time to park and walk to the testing location.

📌 Important Guidelines

• Eligibility: You must be planning to apply to the Saddleback College Nursing Program to reserve a seat for this exam.
• First-Time Test Takers Only: This complimentary ATI TEAS exam is available only to students who have never previously taken the ATI TEAS exam.
• Limited Availability: Seats are limited and will be assigned on a first-come, first-served basis. The reservation form will close once all seats have been filled.
• Legal Name: Enter your legal first and last name exactly as they appear on your government-issued photo identification. Do not enter a preferred name or nickname.
• Identification Required: On the day of the exam, you must bring a valid, physical, government-issued photo ID, such as a driver’s license or passport. The name on your identification must match the name on your ATI account.
• Testing Accommodations: If you require testing accommodations, you must have an approved accommodation plan through Saddleback College Disabled Students Programs and Services (DSPS). Contact DSPS before submitting this reservation form so there is enough time to review and coordinate your approved accommodations. Accommodations cannot be arranged on the day of the exam.

• Confirmation Process: A reservation confirmation will be sent through Submittable within three to five business days. Weekends and holidays are not included.
• Important: If you do not receive a confirmation, don't assume that your reservation has been accepted. Please email scnursing@saddleback.edu to follow up.

⏳ Complete Before the Exam

Complete the following requirements as soon as possible and no later than five days before the exam:

• Apply to Saddleback College if you don't already have a Saddleback College student ID and email address. Select the Fall 2026 term when completing the application:  Saddleback College – How to Apply
• Create an ATI account if you don't already have one:  Create an ATI Account
• Use only your Saddleback College email address for your ATI account. ATI accounts created with a personal or other email address will not work for Saddleback College’s ATI TEAS exam.
• Review your ATI account information. Make sure your legal name, date of birth, and all demographic information are complete and accurate. Your name must match your government-issued photo ID.
• Remember your ATI login information. You will need the username and password you created for your ATI account to sign in on exam day.

Re-Entry Petition Guidelines

The Re-Entry Petition is intended only for students who were previously enrolled in the Saddleback College Nursing Program and are requesting re-entry into the program.

Who Should Submit This Petition?

• Students who were previously enrolled in the Saddleback College Nursing Program.
• Students requesting re-entry following an approved leave of absence.

Important Information

• Submission of a Re-Entry Petition does not guarantee re-entry into the Nursing Program.
• Re-entry is contingent upon space availability and fulfillment of all program requirements.
• Only complete applications with all required documentation will be considered.

Required Documents

Please submit the following:

• Completed Re-Entry Petition Form
• Completed Remediation Plan Form
• Any additional documentation or requirements outlined in your remediation plan

Submission Deadlines

All required materials must be submitted by:

• Spring Entry: November 1, 2026, by 11:59 p.m.
• Fall Entry: April 1, 2027, by 11:59 p.m.

Late or incomplete petitions may not be reviewed.

Notification

Applicants will be notified of the Re-Entry Petition decision approximately 2–3 weeks after the submission deadline.
